The Implementation of Web-Based PatientRegistration System at X Hospital

ABSTRACTRegistration of outpatients at X Hospital is carried out manually. Patients must register by queuing at the registration counter, this will take a long time and be ineffective. The purpose of this study was to implement a web-based outpatient registration information system. The research method used was the waterfall method that consists of the planning, system design, programming, testing, implementation, and maintenance stages. The result of the system created was a web-based outpatient registration system. The results showed that the use of this system can assist patients in registering outpatients. Patients will find it easier to register for outpatient services by using website support technology. In conclusion, the use of a web-based outpatient registration system can help X Hospital in providing health services to patients.

1.INTRODUCTION

A hospital is a health service facility that provides comprehensive outpatient, inpatient, and emergency services [1], [2]. One indicator of a hospital's success is the number of outpatients, which reflects the public's trust in conducting medical examinations there. However, registration of outpatients at X Hospital is still carried out manually.

Patients must register and queue at the registration unit before receiving health services, leading to long wait times and inefficiencies [3]. Additionally, patients often lack information about doctors' practice schedules, which can only be obtained upon arriving at the hospital. This level of information management is inadequate for hospital management and poses an obstacle to hospital development [4].

Therefore, it is necessary to improve the management of outpatient registration at X Hospital to reduce patient waiting times and increase access to necessary information. The X Hospital must evaluate the length of time patients wait before being served, as minimum service standards for hospitals stipulate that outpatient waiting time should not exceed 60 minutes [8].

The implementation of a web-based online outpatient registration system is a viable solution to overcome the problems in outpatient services at X Hospital. The urgency of this research is underscored by the fact that such a system is a requirement for hospital accreditation assessment, as outlined in the Access to Hospital and Continuity of Services standard [9]. With this system, X Hospital can achieve higher marks in the National Hospital Accreditation Standard (SNARS 1), and the registration system can enhance the quality of outpatient services [10].

Furthermore, this system is beneficial for assisting individuals in registering for health services at the hospital, checking doctors' practice schedules, and reducing patient waiting times, thereby mitigating the potential for Covid-19 transmission, given that hospitals are a cluster for Covid-19 transmission.

The purpose of this study is to implement a web-based outpatient registration system based on SNARS 1 at X Hospital. The research method employed is the waterfall approach, which includes steps such as planning, system design, programming, testing, and implementation and maintenance [11]. The implementation of this information system aligns with the vision and mission of X Hospital, which is to realize an excellent, professional, and modern hospital in the field of health services, providing infrastructure facilities accordingly.with technological developments, and develop effective service systems and procedures.

2. RESEARCH METHOD

This research utilized the waterfall method, which comprises several stages: analysis, design, implementation, testing, deployment, and maintenance. However, this article focuses solely on the implementation stage of a web-based patient registration system at X Hospital. Implementation involves creating the system based on the design established in previous stages. The web-based system was implemented using the PHP programming language with the CodeIgniter 4 Framework. CodeIgniter 4 is a leading framework in web-based application development, renowned for its security and speed advantages.

3. RESULTS

The web-based outpatient registration system was developed to facilitate patient registration for health services, accommodate doctors' practice schedules, and minimize waiting times in hospitals, thereby reducing the risk of Covid-19 transmission. System implementation was executed according to the design results from previous stages. The outcomes of implementing the web-based patient registration system are detailed below.

3.1. Patient Register Page Display

The register form is a form that must be filled out when a patient registers for health services at the hospital. The patient registration form consists of registration of new patients at their own request, outpatients, and emergency patients. Based on Figure 1, to register as a new patient at his own request, outpatient, and emergency patient, the patient must enter some data. The data includes the identity card number, patient's name, place of birth, date of birth, last education, religion, and home address. Filling out the outpatient registration form is shown in the following figure.

The next step is to choose a health clinic service. There are several health services options at the clinic

including THT clinics (health services for ears, nose and throat), obstetrics clinics, dental clinics, surgery clinics, internal medicine clinics, eye clinics, ECG (electrocardiography) clinics, pediatric clinics, general clinics, and a VCT (Voluntary Counseling and Testing) clinic.

The next step is for the patient to write down his health problem, for example, the patient vomits in the morning. Then the system will display the registration fee and the patient pays the registration fee by transfer to the hospital account number. The next step is for the patient to upload proof of the transfer of the registration fee in the form provided.

The next step is for the patient to review the data that has been entered. This is shown by the following Figure.

If the entered data is accurate, the patient can proceed to select the submit option. Upon submission, the patient's data will be recorded in the hospital's queue system. Additionally, the hospital administration will review and verify the patient data registered online.

3.2 Queue Management System

This menu is designed specifically for registration officers. Through this menu, officers can validate registrant data via the "antrian pasien" option. To initiate the verification process, officers simply click on the respective option, and the patient will receive their queue number information through their user page. This process is illustrated in the following figure.

3.3. Patient Data Management

Registration officers can perform data management through the "Data Pasien" menu. Through this feature, officers can change data and view patient registration history.

After the implementation stage, the next stage is the testing stage. At the testing stage, the system functionality was tested using blackbox-testing. The test results can be seen in the following table.

Deployment, Maintenance, and Conclusion

The deployment stage involves installing the system on a server at X Hospital. During this stage, data from the legacy system is uploaded and imported into the new system, particularly for previous patient records. The final stage of the Waterfall method is the maintenance stage, where the implementation of the system is monitored. Prompt repairs are conducted in response to any system issues.

Conclusion

The patient registration system at X Hospital was developed using the PHP programming language with the CodeIgniter 4 framework. The Waterfall method was employed for system development, proving effective for this purpose. A web-based patient registration system enhances health services and patient satisfaction at X Hospital.

The system underwent blackbox testing to assess feature functionality, resulting in satisfactory performance. However, further testing is required regarding system security to ensure the protection of patient data.

X医院基于Web的患者登记系统的实现

摘要:X医院门诊患者的登记是手动进行的。患者必须在登记柜台排队登记,这将需要很长时间,而且无效。本研究旨在实现一个基于网络的门诊挂号信息系统。使用的研究方法是瀑布法,包括规划、系统设计、编程、测试、实施和维护阶段。创建该系统的结果是一个基于网络的门诊登记系统。结果表明,使用该系统可以帮助患者登记门诊。患者会发现使用网站支持技术更容易注册门诊服务。总之,使用基于网络的门诊挂号系统可以帮助X医院为患者提供医疗服务。

1.简介

医院是一种提供全面门诊、住院和急诊服务的医疗服务设施[1],[2]。医院成功的一个指标是门诊人数,这反映了公众对在那里进行体检的信任。然而,X医院的门诊登记仍然是手动进行的。

患者在接受医疗服务之前必须在登记处登记和排队,导致等待时间长,效率低[3]。此外,患者往往缺乏有关医生执业时间表的信息,这些信息只能在到达医院后才能获得。这种信息管理水平不足以满足医院管理,并对医院发展构成障碍[4]。

因此,有必要改善X医院的门诊挂号管理,以减少患者等待时间,增加获取必要信息的机会。X医院必须评估患者等待服务的时间长度,因为医院的最低服务标准规定门诊等待时间不应超过60分钟[8]。

实施基于网络的在线门诊挂号系统是克服X医院门诊服务问题的可行解决方案。正如《医院准入和服务连续性标准》[9]所述,这一系统是医院认证评估的要求,这一事实突显了这项研究的紧迫性。通过该系统,X医院可以在国家医院认证标准(SNARS 1)中获得更高的分数,注册系统可以提高门诊服务的质量[10]。

此外,鉴于医院是新冠肺炎传播的聚集地,该系统有利于帮助个人在医院注册医疗服务,检查医生的执业时间表,减少患者等待时间,从而降低新冠肺炎传播的可能性。

本研究的目的是在X医院实施一个基于SNARS 1的基于网络的门诊挂号系统。所采用的研究方法是瀑布法,包括规划、系统设计、编程、测试以及实施和维护等步骤[11]。该信息系统的实施符合X医院的愿景和使命,即在医疗服务领域实现卓越、专业和现代化的医院,根据技术发展提供基础设施,并开发有效的服务系统和程序。

2.研究方法

本研究采用瀑布法,包括几个阶段:分析、设计、实现、测试、部署和维护。然而,本文仅关注X医院基于网络的患者登记系统的实施阶段。实施涉及根据前几个阶段建立的设计创建系统。基于web的系统是使用PHP编程语言和CodeIgniter 4框架实现的。CodeIgniter4是基于web的应用程序开发中的领先框架,以其安全性和速度优势而闻名。

3.结果

开发基于网络的门诊登记系统是为了方便患者登记医疗服务,适应医生的执业时间表,并最大限度地减少医院的等待时间,从而降低新冠肺炎传播的风险。系统实现是根据前几个阶段的设计结果执行的。实施基于网络的患者登记系统的结果详述如下。

3.1. 患者登记页面显示

登记表是患者在医院登记医疗服务时必须填写的表格。患者登记表包括根据自己的要求登记新患者、门诊患者和急诊患者。根据图1,要根据自己的要求注册为新患者、门诊患者和急诊患者,患者必须输入一些数据。这些数据包括身份证号码、患者姓名、出生地、出生日期、最后受教育程度、宗教信仰和家庭住址。填写门诊挂号表如下图所示。

下一步是选择健康诊所服务。诊所提供多种医疗服务选项

包括THT诊所(耳鼻喉健康服务)、产科诊所、牙科诊所、外科诊所、内科诊所、眼科诊所、心电图诊所、儿科诊所、普通诊所和VCT(自愿咨询和检测)诊所。

下一步是让患者写下他的健康问题,例如,患者早上呕吐。然后,系统将显示注册费,患者通过转账到医院账号支付注册费。下一步是患者在提供的表格中上传注册费转账证明。

如果输入的数据准确,患者可以继续选择提交选项。提交后,患者的数据将记录在医院的队列系统中。此外,医院管理部门将审查和核实在线登记的患者数据。

3.2队列管理系统

此菜单是专门为登记官设计的。通过此菜单,官员可以通过“antrian pasien”选项验证注册人数据。要启动验证过程,官员只需点击相应的选项,患者将通过其用户页面收到队列号信息。下图展示了这一过程。

3.3. 患者数据管理

注册官可以通过“数据通道”菜单进行数据管理。通过此功能,官员可以更改数据并查看患者登记历史。

在实施阶段之后,下一阶段是测试阶段。在测试阶段,使用黑体测试对系统功能进行了测试。测试结果见下表。

部署、维护和结束

部署阶段涉及在X医院的服务器上安装系统。在此阶段,传统系统的数据被上传并导入新系统,特别是之前的患者记录。瀑布法的最后阶段是维护阶段,在此阶段对系统的实施进行监控。及时维修以应对任何系统问题。

结论

X医院的患者登记系统是使用PHP编程语言和CodeIgniter 4框架开发的。瀑布法用于系统开发,证明了其有效性。基于网络的患者登记系统提高了X医院的医疗服务和患者满意度。

该系统经过了黑盒测试,以评估特征功能,取得了令人满意的性能。然而,需要对系统安全性进行进一步测试,以确保患者数据的保护。
